Eligible applicants must be an elderly person and/or an individual with a disability. The primary goal for inclusion of any
property in this program must be modifications needed due to age or disability. The owner or renter of the property to be
repaired most be income eligible and must agree to the terms of assistance. Application must be completed by owner of home.

Before You Visit

    The first step in this program begins with a referral from Options Center for Independent Living or Catholic Charities. You can contact them via information below:

application, call (815)936-3567

STEPS:

    1. Referral from Agency (see "Before You Visit")
    2. ECDA employee will schedule a house visit to assess the eligibility of the home needing assistance.
    3. Application will be given (in office or mailed to the home) once approved for assistance.
    4. Once ECDA receives the completed application, we will begin the coordination with our approved contractors to complete the work.

Required Documents:

    1. Completed Application
      1. Authorization for Release of Information
      2. IRS 4506T Form
      3. Two (2) months bank statements (no exceptions)
    2. Proof of Income (2 months)
      1. IRS 1040, W2, and check stubs
      2. Income Tax Return (2 years)
      3. Needed for ALL adults living in the home
    3. Proof of Ownership - Warranty deed, quit claim deed or other deed information (mortgage papers)
      1. with legal description
      2. proof that mortgage payments are current
    4. Property Record Card - Get from County Building, 189 E. Court Street
    5. Proof of Insurance - Certificate of Insurance from your insurance agent
    6. Proof of Paid Taxes - Copy of your current tax bill
    7. Doctor's Note

    You must return all of the above listed documents for your application to be processed.
