Single Family Rehab allows families who meet HUD income eligibility guidelines to apply for assistance to repair areas in their
home which have been identified as a potential threat to the maintenance of the residence and are not in compliance with
HUD's Housing Quality Standards and local code requirements. An inspection is performed by a qualified staff member and
determination of feasibility is then evaluated. The home must be the principal residence of the owner and the owner must
agree to the terms of assistance.

Required Documents:

    1. Completed Application
      1. Authorization for Release of Information
      2. IRS 4506T Form
      3. Two (2) months bank statements (no exceptions)
    2. Proof of Income (2 months)
      1. IRS 1040, W2, and check stubs
      2. Income Tax Return (2 years)
      3. Needed for ALL adults living in the home (owners or tenants)
    3. Proof of Ownership - Warranty deed, quit claim deed or other deed information (mortgage papers)
      1. with legal description
      2. proof that mortgage payments are current
    4. Property Record Card - Get from County Building, 189 E. Court Street
    5. Proof of Insurance - Certificate of Insurance from your insurance agent
    6. Proof of Paid Taxes - Copy of your current tax bill

    You must return all of the above listed documents within 10 business days for your application to be processed.
